What is wrong?

Describe the problem

Between 20 and 2,000 characters. Tell us what is incorrect and, if you know it, what the correct information is. Do not include personal health details.

Your email address (optional)

Only used if we need to ask you a follow-up question. Never shown publicly.

By sending this report you agree to our terms and privacy policy.

What happens next

Your report goes to the HealthNaro review queue with the page you named attached.
We check the detail against the provider or a public source. Most reports are handled within a few working days.
If the information is confirmed wrong, the profile is corrected or taken down. Reports never change a provider's verification level by themselves.

Something else?

Is this your own profile? Claim it to edit the details yourself.
